The deadline for submitting a reimbursement claim for 2021 is April 30th!

Please make sure that we have your correct address by contacting

801-363-0815 Monday Through Friday 8 AM to 4 PM.

I will be at the Local 99 Union Hall

Mondays from around 10:30 AM to 12:30 PM with the exception of Holidays.

In order to file a claim for any services, you must have current valid Health Insurance!

Annual Notifications will be mailed out by March 15th, 2022.
